Enrol Your Child/Children

Information that you may require to register your child/children:

  • Customer Reference Number (CRN) for the parent registered for family payments at Dept. Human Services
  • Customer Reference Number (CRN) for EACH child to be registered for care
  • An up-to date Immunisation History Statement for ALL non-school aged children
  • Email addresses for ALL parent/guardians and Authorised people (a PIN number will be emailed for e-signing)
  • Authorised persons contact details
  • Medical details inc. medicare number, doctor's contact and health insurance
  • Medical Plan for medical conditions or allergies. e.g. asthma or anaphylaxis
  • Custody Court Orders
  • Birth Certificates
